Engineering 

Supplementary material required: Prior to approval of the conditional use application, the owner shall obtain a permit to plug each existing sanitary sewer lateral that serves a building that is proposed for demolition. For each lateral to be plugged the owner shall deposit $1,000 with the City Engineer in two separate checks in the following amounts: (1). $100 non-refundable deposit for the cost of inspection of the plugging by City staff; and (2). $900 for the cost of City crews to perform the plugging. If the owner elects to complete the plugging of a lateral by private contractor and the plugging is inspected and approved by the City Engineer, the $900 fee shall be refunded to the owner.

Supplementary material required: Fee for erosion control permit: $285.15. Submit to Jeff Benedict payable to City of Madison Treasurer.

Supplementary material required: Fee for stormwater management permit: $5 per 1000 sf new impervious area (not just added impervious area). Submit to Jeff Benedict payable to City of Madison Treasurer.

Supplementary material required: The Applicant shall submit prior to sign-off, digital PDF files to the Engineering Division (attention: Jeff Benedict, email: jbenedict@cityofmadison.com). Engineering Mapping 

Supplementary material required: The Applicant shall submit, prior to plan sign-off, a digital CAD file (single file) to the Engineering Program Specialist in the Engineering Division (Lori Zenchenko). The digital CAD file shall be to scale and represent final construction. The single CAD file submittal can be either AutoCAD (dwg) Version 2001 or older, MicroStation (dgn) Version J or older, or Universal (dxf) format and contain the only the following data, each on a separate layer name/level number: a) Building Footprints; b) Internal Walkway Areas; c) Internal Site Parking Areas; d) Other Miscellaneous Impervious Areas (i.e. gravel, crushed stone, bituminous/asphalt, concrete, etc.); e) Right-of-Way lines (public and private); f) All Underlying Lot lines or parcel lines if unplatted; g) Lot numbers or the words “unplatted”; h) Lot/Plat dimensions; i) Street names. All other levels (contours, elevations, etc) are not to be included with this file submittal. NOTE: Email file transmissions preferred Lzenchenko@cityofmadison.com . Include the site address in the subject line of this transmittal. Any changes or additions to the location of the building, sidewalks, parking/pavement during construction will require a new CAD file.

Supplementary material required: City Engineering records indicate that an 8" public sanitary sewer main exists, and the civil site plan identifies this public sanitary main and sewer access structure southeast of the existing brick wall dumpster enclosure. City Engineering does not have record of a recorded public sanitary sewer easement, as this sewer was built in circa 1950 while the property was still in the Town of Blooming Grove. Applicant shall provide a copy of a recorded easement, if one in fact exists, most likely conveyed to the Town prior to annexation to the City of Madison. If an easement of records cannot be found, the current owner shall convey an easement to the City prior to plan approval. Fire 

Redraft required: The revised site plans shall clearly identify the location of all fire lanes, the fire lanes as identified on this plan will not work they are to narrow at places and also do not make the turning radius. You may consider showing the fire lane in different location, call Scott 261-9843 for details.

Supplementary material required: Too much trespass light. Please submit one copy of revised photometric plot with fixture cutsheets.

Planning 

Supplementary material required: The applicant shall submit the floorplans and four-sided archiectural plans for review and approval by the Planning Division and Urban Design Commission staff. These items shall reflect the elevations approved by the UDC and Plan Commission in October/ November 2007 per staff approval.

Supplementary material required: The Driveway Approach form is already filled out in Traffic Engineering Office and requires only the fee and deposit to be paid as noted prior to site approval. Supplementary material required: Need to provide detailed building elevations including references to building materials and color selection

Zoning 

Redraft required: The accessible stalls do not meet code requirements. Access aisles shall extend the full length of the parking spaces they serve.

Supplementary material required: Provided bicycle parking is improper. Refer to MGO sections: 28.11(3)(e), 28.11(3)(h) 2. d and 28.11(3)(i) 3 for information on properly providing bicycle parking.
