Provide details on site plan to clearly show the amount of added impervious area and the amount of disturbed area. Over 4,000 sf disturbed requires and Erosion Control & Stormwater Management permit. Over 20,000 sf disturbed requires an erosion control plan. Application form can be found on the City's website at http://www.cityofmadison.com/engineering/documents/ErosionPermit.pdf. Include applicable permit fees and a Universal Soil Loss Equation Worksheet (USLE).

Engineering Mapping
Supplementary material required:
Submit a complete PDF representing the final city approved site plan for all existing and new buildings within the Amended SIP , including all floor levels and layouts, to Lori Zenchenko ( Lzenchenko@cityofmadison.com ) to coordinate and obtain an approved final addressing plan for the resultant SIP site. If there are any changes following final city approvals, pertaining to the location of a unit, the deletion or addition of a unit, or to the location of the entrance into any unit, (before, during or after construction) the addresses may need to be changed. Resubmittal of any changes shall be made to determine if the address plan needs to be changed. The interior address plan is subject to the review and approval of the Fire Marshal.
Supplementary material required:
The Applicant shall submit, prior to plan sign-off, a digital CAD file (single file) to the Engineering Program Specialist in the Engineering Division (Lori Zenchenko). The digital CAD file shall be to scale and represent final construction. The single CAD file submittal can be either AutoCAD (dwg) Version 2001 or older, MicroStation (dgn) Version J or older, or Universal (dxf) format and contain the only the following data, each on a separate layer name/level number: a) Building Footprints; b) Internal Walkway Areas; c) Internal Site Parking Areas; d) Other Miscellaneous Impervious Areas (i.e. gravel, crushed stone, bituminous/asphalt, concrete, etc.); e) Right-of-Way lines (public and private); f) All Underlying Lot lines or parcel lines if unplatted; g) Lot numbers or the words “unplatted”; h) Lot/Plat dimensions; i) Street names. All other levels (contours, elevations, etc) are not to be included with this file submittal. NOTE: Email file transmissions preferred Lzenchenko@cityofmadison.com . Include the site address in the subject line of this transmittal. Any changes or additions to the location of the building, sidewalks, parking/pavement during construction will require a new CAD file.
Supplementary material required:
The existing Condominium Plat and Declaration documents shall be amended as required by Chapter 703 WI. Statutes.

Fire
Redraft required:
Provide a completed MFD "Fire Apparatus Access and Fire Hydrant Worksheet" with the site plan submittal.

Provide fire apparatus access as required by MGO 34.19, IFC 503 and IFC Appendix D105.

Traffic Engineering
Redraft required:
When the applicant shall submit plans for approval, the applicant shall show the following: addresses, one contiguous plan, all pavement markings, building placement, all driveway approaches at D’Onofrio Dr and Zor Shrine Place, signage, dimensions of aisles, driveways, stalls including the two (2) feet overhang, and a scaled drawing at 1" = 40'.
Redraft required:
"Stop" signs shall be installed at a height of seven (7) feet at all driveway approaches behind the property line and noted on the plan. All directional/regulatory signage and pavement markings on the site shall be shown and noted on the plan.
Redraft required:
The applicant shall show the dimensions for existing parking stalls’ items B, C, D, E, and F, and degree of angle parking width and backing up, according to Figures II "Medium and Large Vehicles" and “Small Vehicles” parking design standards. Signs and planting areas are to be excluded from the rectangular stall areas including the two (2) feet of vehicle overhang. The two (2) feet of vehicle overhang shall be shown on the plan and dimensioned.
Redraft required:
This site was approved with “Small Vehicles” that will need to be shown and dimensioned. Per ordinance, the small car stalls shall not exceed 25% of the total number of Medium and Large Vehicles and Small Vehicles stalls for the facility. The site plan shall show small car parking spaces identified and properly controlled with a sign “Small Cars Only” per each space, when plans are submitted for approval.
Redraft required:
The parking facility shall be modified to provide for adequate internal circulation for vehicles. The site plan shall be modified to accommodate internal circulation for the parking west of Building noted as “One Point Place.” The applicant shall add a 20 ft drive aisle “F” according to M.G.O.
Redraft required:
The applicant shall provide pavement markings for all pedestrian crossings in the parking lot. The applicant should also consider additional measures, which would further enhance pedestrian pathways/linkage from the public sidewalk to the building entrances.The applicant shall show the pedestrian linkage to as noted to D’Onofrio Drive.
Redraft required:
The applicant shall show all bike racks and type of rack installed. The bike parking facilities shall be located in a safe and convenient location in front of the building. The bike rack shall be securely anchored to the ground or building to prevent the rack from moving and be shown on the plan.
