Note: A permit to connect to the public storm sewer and to work in the public right-of-way will need to be obtained by the contractor(s) prior to making the storm & water connections. The contractor(s) will need to be a pre-approved contractor in order to obtain this permit. Depending on the rating of the street at the time the permit is obtain there may additional street patching/repair requirements than those shown on the proposed plan. The permit application and instructions can be found on City Engineering's website at http://www.cityofmadison.com/engineering/Permits.cfm. The application used for this permit is the "Excavate in the Right-of-Way" permit.

Note: Supplementary Condition Completed (was The Erosion Control & Storm Water Management Permit Application submitted did not include the 16,200 sf disturbed area in the fee calculation. Note: Supplementary Condition Completed (was The Storm Water Management Maintenance Agreement that was submitted was editted to the effect that the agreement would be automaticlly terminated if the owner no longer owns the property. This is not acceptable as the requirements must transfer to the new owner (which is covered in 4.(d) of the document). A new SWMA must be completed and submitted without this editted language prior to plan sign-off. Contact Tim Troester at 608-267-1995 to obtain the standard language for this agreement and applicable information. The original signed/notarized agreement will need to be submitted to City Engineering (which will then be recorded with Dane County Register of Deeds) prior to plan sign-off.)

Note: Supplementary Condition Completed (was Supplementary Condition Completed (was The Applicant shall submit, prior to plan sign-off, a digital CAD file (single file) to the Engineering Program Specialist in the Engineering Division (Lori Zenchenko). The digital CAD file shall be to scale and represent final construction. The single CAD file submittal can be either AutoCAD (dwg) Version 2001 or older, MicroStation (dgn) Version J or older, or Universal (dxf) format and contain the only the following data, each on a separate layer name/level number: a) Building Footprints; b) Internal Walkway Areas; c) Internal Site Parking Areas; d) Other Miscellaneous Impervious Areas (i.e. gravel, crushed stone, bituminous/asphalt, concrete, etc.); e) Right-of-Way lines (public and private); f) All Underlying Lot lines or parcel lines if unplatted; g) Lot numbers or the words “unplatted”; h) Lot/Plat dimensions; i) Street names.
