Redraft required:
The inlet filters proposed for use on this project for oil/grease treatment are not acceptable. The design is more for the temporary containment of a spill then for permanent control in a parking lot setting. The exposed fabric will be prone to tearing or vandalism. Any filter unit/device should be contained within the proposed inlet and provide treatment for removal of oil/grease from the parking lot runoff. Revise detail to show an acceptable unit that is contained within the inlet.
Supplementary material required:
Submit a Stormwater Management Report (stamped by a P.E. licensed in the State of Wisconsin) that details how the stormwater management requirements for this site are being met. Provide any applicable calcs, product information, etc
Supplementary material required:
The applicant shall submit, prior to plan sign-off, digital PDF files to the City Engineering Division. The digital copies shall be to scale, shall have a scale bar on the plan set, and shall contain the following items: building footprints; internal walkway areas; internal site parking areas; lot lines and right-of-way lines; street names, stormwater management facilities and; detail drawings associated with stormwater management facilities (including if applicable planting plans). Supplementary material required:
Prior to approval of the conditional use application, the owner shall obtain a permit to plug each existing sanitary sewer lateral that serves a building that is proposed for demolition. For each lateral to be plugged the owner shall deposit $1,000 with the City Engineer in two separate checks in the following amounts: (1) $100 non-refundable deposit for the cost of inspection of the plugging by City staff; and (2) $900 for the cost of City crews to perform the plugging. If the owner elects to complete the plugging of a lateral by private contractor and the plugging is inspected and approved by the City Engineer, the $900 fee shall be refunded to the owner. Supplementary material required:
The Applicant shall submit, prior to plan sign-off, a digital CAD file (single file) to the Engineering Program Specialist in the Engineering Division (Lori Zenchenko). The digital CAD file shall be to scale and represent the final approved plan and construction. The single CAD file submittal can be either AutoCAD (dwg) Version 2001 or older, MicroStation (dgn) Version J or older, or Universal (dxf) format and contain the only the following data, each on a separate layer name/level number: a) Building Footprints; b) Internal Walkway Areas; c) Internal Site Parking Areas; d) Other Miscellaneous Impervious Areas (i.e. gravel, crushed stone, bituminous/asphalt, concrete, etc.); e) Right-of-Way lines (public and private); f) All Underlying Lot lines or parcel lines if unplatted; g) Lot numbers or the words “unplatted”; h) Lot/Plat dimensions; i) Street names. All other levels (contours, elevations, etc) are not to be included with this file submittal. NOTE: Email file transmissions preferred Lzenchenko@cityofmadison.com . Include the site address in the subject line of this transmittal. Any changes or additions to the location of the building, sidewalks, parking/pavement during construction will require a new CAD file.

The applicant has responded to condition #32a, which requested the shutters shown on the northern and southern elevations to have vision glass behind them where not located in a restroom, office or kitchen. The shutters were to be operable with vision glass beyond to allow natural lighting into the restaurant. The applicant responded that all but one of the shutters shown would be in a restroom, office or kitchen and that the last set of shutters would be in a bar area, where having a window would create an undue burden on the operation of the restaurant. Based on the July 1, 2009 letter from the applicant, Planning staff will accept the elevations as submitted with the inoperable/ decorative shutters as shown.

The owner shall provide a no cost easement to the City for the placement of City-owned traffic signal facilities at the Gammon Road-Gammon Place intersection, including, but not limited to, the right of ingress and egress; the right to excavate, install, operate, maintain, repair, replace, and modify the facilities; and the right to perform all work incidental thereto upon and over the required easement area. The applicant shall provide a “Perpetual Limited Easement for Public Traffic Signal Purposes” to Jeff Ekola, City of Madison Real Estate prior to final sign off.
