Supplementary material required: All outstanding Madison Metropolitan Sewerage District (MMSD) and City of Madison sanitary sewer connection charges are due and payable prior to plan approval.

Supplementary material required: Need notary signed maintenance agreement by owner for the bioretention basins. Contact Jeff Benedict for boilerplate and Exhibit A language.

Supplementary material required: Need ROW permit for extension of 66 inch RCP storm sewer and 66 inc RCP culvert under drive openings.

Fire 

Note: Per MGO 34.20, The following requirements are in addition to the requirements in IFC Section 508.1:(1) Fire Protection Water Supply and Fire Hydrants.(a) Private fire hydrants and water mains shall be installed in accordance with NFPA24, and the City of Madison Standard Specifications for Public Works Construction,Sec. 704.7. Private fire hydrants and water mains shall be maintained in accordance with NFPA 25. (e) Minimum Flow and System Design.1. Private fire service mains serving fire hydrants shall be the same size as the public mains supplying the private water system unless hydraulically calculated to provide the minimum required flow and pressure in accordance with nationally recognized standards. In all cases, mains do not need to exceed 10 inches but shall be at least six inches in diameter. You need to Confirm the size of the street main and install the correct pipe size to the new hydrants. 2. Control valves shall be provided to limit the number of private hydrants affected by maintenance, repair or construction. There is one shut off valve for 2 hydrants, there shall be one shut off for each hydrant.
