Engineering
Supplementary material required:
per Comments 11 & 19, Conditions of Approval Letter, Sept 2, 2009: Provide signed & recorded copy of agreement covering stormwater discharge/easements across lot lines. A draft copy was submitted at the PUD/GDP phase that was acceptable to Engineering (if not recorded already it must be completed and a copy submitted to City Engineering prior to plan sign-off)

per Comment 13, Conditions of Approval Letter, Sept 2, 2009: Submit copy of recorded ownership/maintenance agreement for the private sanitary. A draft copy was submitted at the PUD/GDP phase that was acceptable to Engineering (if not recorded already it must be completed and a copy submitted to City Engineering prior to plan sign-off)
Supplementary material required:
A Stormwater Management Maintenance Agreement must be completed, signed & notarized prior to plan sign-off. The draft provided in the SWM report is acceptable provided all of the parcel numbers for the Midtown Center Plat are added. It must be completed, signed, notarized and submitted to Engineering for recording prior to plan sign-off. This should be completed by the plat developer, but if any parcels have already been sold the current owners will also need to be included and the document added to include signature areas for them as well.
Supplementary material required:
An Erosion Control & Stormwater Management Permit will be required for this project. Submit a completed Erosion Control Permit & Stormwater Management Permit application form, USLE worksheet, and erosion control plan (plan must be stamped by a P.E. licensed in Wisconsin and provided as 11x17 and PDF). The permit application can be found on the City’s website at http://www.cityofmadison.com/engineering/Permits.cfm.

Supplementary Condition Completed (was per Comment 15, Conditions of Approval Letter, Sept 2, 2009: All impact fees for the Upper Badger Mill Creek Impact Fee District are due and payable prior to issuing building permits. Engineering Mapping
Supplementary material required:
When all pending private stormwater and/or sanitary sewer documents (easements and maintenance agreements) are approved by the City Engineer and recorded with the Dane County Register of Deeds, provide recorded copies to City Engineering and Engineering Mapping. Draft documents were previously prepared by Reinhart, Boerner & Van Deuren and approved per form, yet never recorded with the Register of deeds. Coordinate review, approval and recording again with City Engineering. Reference current Engineering Project No. 53B2871.

Supplementary Condition Completed (was This site plan has reconfigured various building door locations. It appears floors 1, 2 and 3 for the building at 8119 Mayo Dr and floors 2, 3 and 4 for the building at 8101 Mayo Dr have been changed. Submit updated PDF files of all final floor plans to Engineering Mapping (addressing@cityofmadison.com ) so that an amended addressing plan can be developed. In the event there are any changes pertaining to the location of a unit, the deletion or addition of a unit, or to the location of the entrance into any unit, (before, during or after construction) the amended plan shall be resubmitted for review and addresses may again need to be changed. The interior address plan is subject to the review and approval of the Fire Marshal.)

Supplementary Condition Completed (was The site plan indicates proposed private sanitary sewer facilities located outside of platted private easements. Per the September 2, 2009 Planning and Development Conditional Approval letter, prepare new or amended private easement documents, inclusive of maintenance agreements, for all affected parties.
